As the parent or official guardian of the children I am registering for Wize 'N Up 2026, I hereby grant permission to Milestones Church, its employees, or representatives to take and use photographs, videotape, and digital images of my child(ren) for use in future promotional materials for Wize 'N Up. I authorize the use of these images indefinitely without compensation to me. All negatives, positives, prints, digital reproduction, and videotape shall be property of Milestones Church.
I hereby acknowledge that I have voluntarily agreed to have my child participate in the Wize 'N Up program. I understand that activities at Wize 'N Up may involve risk and I, for my child, willingly assume the risk or injury as my responsibility. As lawful consideration for my child being permitted to participate in the Wize 'N Up program by Milestones Church, I release from any legal liability whatsoever Milestones Church and all members, employees, agents or volunteers for any and all injury caused by or resulting from my child's participation in Wize 'N Up. I indemnify and hold harmless and also agree not to sue, claim against, attach the property or prosecute Milestones Church for any injury caused by or resulting from my child's participation in the activities at Wize 'N Up. I am aware that I am releasing certain legal rights that I otherwise may have and I enter into this release on behalf of myself and/or my child of my own free will. I understand that my child must be entering first grade or higher and cannot be older than entering 6th grade to participate in the activities of Wize 'N Up.
