  • Mandatory Reporting Guidelines

  • 1. Legal Authority & Mandated Reporter Status

  • Pursuant to Arizona Revised Statutes § 13-3620, any person who reasonably believes that a minor is or has been the victim of abuse or neglect shall immediately report or cause a report to be made to a peace officer or Child Protective Services (CPS).
  • For purposes of these Guidelines:
    • A minor is any individual under the age of 18.
    • Success Coaches, volunteers, mentors, and school personnel working with students at Edge High School are considered mandatory reporters under Arizona law because they have responsibility for the care, supervision, or treatment of minors.
  • Failure to comply with mandatory reporting laws may result in disciplinary action, removal from the Success Coaching Program, and civil or criminal penalties as provided by law.
  • 2. Reasonable Belief Standard

  • A report is required when a coach or volunteer forms a reasonable belief that abuse or neglect may have occurred. A reasonable belief may arise from, but is not limited to:
    • A student discloses information indicating physical abuse, sexual abuse, neglect, or emotional abuse
    • A student presents with unexplained injuries, or explanations that are inconsistent with observed injuries
    • A third party (peer, sibling, caregiver, or staff member) provides information suggesting abuse or neglect
  • You are not required to confirm or prove abuse. The law requires reporting based on reasonable suspicion only.
  • 3. Types of Abuse and Neglect That Must Be Reported

  • Physical Abuse
  • Any physical injury, threatened injury, or mental injury inflicted on a child by a person responsible for the child's care.
  • Sexual Abuse
  • Any act or threatened act of sexual conduct involving a child, including exploitation, by a caregiver or a person in a position of authority.
  • Emotional Abuse
  • Patterns of behavior or attitudes that interfere with a child's emotional development, mental health, or social functioning.
  • Neglect
  • Failure to provide necessary food, shelter, clothing, supervision, or medical care; or failure to protect a child from conditions that seriously endanger physical or mental health when reasonably able to do so.
  • 4. How and Where to Report

  • Reports should be made immediately and may be made confidentially to one or more of the following:

  • Law Enforcement

    • Local non-emergency law enforcement line
    • Call 911 if the child is in immediate danger

  • If the alleged abuser is a certificated educator:

  • 5. Internal Notification (Does Not Replace Reporting)

  • In addition to making a report to CPS or law enforcement, Success Coaches must promptly notify an Edge High School administrator or designee that a report has been made.
  • Important: Notifying a school administrator does not satisfy your legal duty to report. The obligation to report rests with the individual who has reasonable belief.
  • 6. What Not to Do

  • Success Coaches and volunteers must not:
    • Conduct their own investigation
    • Ask leading or probing questions
    • Promise confidentiality to a student
    • Delay reporting
    • Discuss the report with unauthorized individuals
  • 7. Investigation Process (Overview)

  • Once a report is made:
    • The investigating agency determines whether the report is substantiated or unsubstantiated
    • Investigations may include interviews with the child, caregivers, and others with relevant information
    • Substantiated cases may result in services or referrals; unsubstantiated cases may be closed
    The reporting individual's role ends after making the report, unless additional information is requested by investigators.
  • 8. Required Training: Video & Quiz

  • All Success Coaches and volunteers are required to complete mandatory reporting training prior to working with students and annually thereafter.

  • Failure to complete the video and quiz may result in delay or denial of authorization to serve as a Success Coach.
  • 9. Confidentiality & Good-Faith Protection

  • Reports made in good faith are confidential and protected under Arizona law. Retaliation against individuals who make reports in good faith is strictly prohibited.
  • 10. Acknowledgment of Understanding

  • I acknowledge that I have received, read, and understand the Mandatory Reporting Guidelines for the Edge High School Success Coaching Program. I understand my legal obligations under Arizona law and agree to comply with all reporting requirements, including completion of required training.
