2026 Summer Camp Continuing Agreement, Waiver, and Release
    May 26 – July 24, 2026

Episcopal Day School, a division and mission of The Church Wardens and Vestrymen of Christ’s Church in Pensacola, Inc. (the “School”), offers weekly summer camps that may include both on-campus and off-campus activities.

To allow your child to participate in the 2026 summer camp program, this Continuing Agreement, Waiver, and Release must be signed and returned before your child attends camp.

This Agreement applies to all camp weeks and related off-campus activities during the 2026 summer season (May 26 – July 24, 2026). It remains in effect for the entire summer unless you withdraw it in writing. If withdrawn, your child will no longer be eligible to participate in remaining 2026 summer camp sessions or related off-campus activities.

By signing below, I/We agree to the following:

    1. Parent/Guardian Confirmation
I/We confirm that I/we am/are the parent(s) or legal guardian(s) of the above-named student (“Student”). I/We understand this agreement is required for participation in summer camp and related activities.

    2. Permission and Assumption of Risk
I/We give permission for the Student to participate in all 2026 summer camp sessions and related off-campus activities.

I/We understand that camp activities involve inherent risks, including the possibility of injury, property damage, disability, and, although rare, death. I/We voluntarily accept and assume all risks related to the Student’s participation.

    3. Emergency Medical Authorization
In the event of an emergency, I/We authorize the School to obtain medical treatment for the Student at my/our expense.

The School will make reasonable efforts to contact a parent/guardian or emergency contact as soon as possible.

    I/We:

Consent to necessary emergency medical care.
Agree to reimburse the School for any medical expenses incurred on the Student’s behalf.
Release and hold the School harmless for decisions made in good faith regarding emergency care.

    4. Student Conduct
I/We understand that the Student must follow all School rules and directions from faculty, staff, coaches, volunteers, and chaperones during camp and related activities.

If the Student does not follow expectations, the School may dismiss the Student from camp without refund. In such cases, transportation home will be the responsibility of the parent/guardian.

The Student must also follow all camp-specific guidelines.

    5. Release of Claims and Indemnification
I/We release and waive any claims against:

    Episcopal Day School,
The Church Wardens and Vestrymen of Christ’s Church in Pensacola, Inc., and
Their officers, directors, employees, staff, volunteers, chaperones, and representatives
(collectively referred to as the “Releasees”) for any injury, loss, or damage arising out of the Student’s participation in summer camp or related off-campus activities. This includes claims related to:
Acts or omissions of the School or its personnel
Actions of third-party providers (such as transportation or venues)
    Reasonable health and safety decisions
Emergency medical care provided under this Agreement

I/We also agree to be responsible for any damage caused by the Student to School property or the property of others.

    6. Limitation of Liability
If any part of this Agreement is found unenforceable, the remaining provisions remain in effect. In any event, the total liability of the Releasees shall not exceed applicable insurance limits, and they shall not be liable for indirect, incidental, consequential, or punitive damages.

This Release applies only to the parties listed above.

    7. Acknowledgment
    I/We confirm that:

I/We have read and understand this Agreement.
    I/We agree to its terms.
I/We understand that information about specific off-campus activities will be provided.
Additional consent may be required for certain activities.

  • Confirmation

Please carefully read the following information. The law requires that we provide certain information to you and obtain your consent to use an electronic signature prior to your doing so.  By electronically signing below, you certify that you have read, understand and agree to the terms of this Disclosure and Consent to Use of an Electronic Signature:

    • You may choose not to consent to use of an electronic signature.  However, online registration requires an electronic signature and the School strongly urges you to consider the benefits, including a more efficient registration process for parents, including registration payment online, better data accuracy, and a more green approach to paper management.
    • By your signature below, you agree and consent to enter into this agreement electronically.
    • If this Disclosure and Consent is executed by only one parent or guardian who is the sole custodian, the one parent or guardian must sign his/her name on BOTH the first and second parent signature lines.  By doing so, you are attesting that you have sole custody of the Student(s), have the authority to make all school-related decisions regarding the Student(s), have the authority to waive claims and to enter into contracts on behalf of the Student(s), and are accepting full responsibility to pay all amounts due under the terms of the registration.
    • You agree to inform the School immediately of any changes in your email address, which you are prompted to update in this online registration process.
    • In order to access and retain the electronic records, you must have a Web Browser and a PDF reader for viewing and printing the document, such as Adobe Reader. You hereby agree that you are able to access and retain the records in this format and you are satisfied with the necessary software and hardware requirements.
    • You agree and consent that your use of a key pad, mouse or other device to select an item, button, icon, checkbox, to enter text, or to perform a similar act/action, (e.g., by typing in your name and clicking on “Submit My/Our Electronic Signature”) constitutes your electronic signature and signifies your intent to be bound.  You understand that your electronic signature is legally binding, just as if you manually signed a paper document in ink.

    ELECTRONIC SIGNATURE

By typing my/our name below and clicking “Save” I/we hereby attach my/our electronic signature to and agree to the terms and conditions of the Consent to Use of Electronic Signature. Your electronic signature is legally binding. You may decline to consent to the use of an electronic signature.  If you choose to not electronically sign and click “save” in this registration form, this will be treated as a cancellation of this transaction, and your child will not be enrolled in the selected classes unless you complete the requisite contracts/forms on paper and return handwritten signed copies to the School by the respective deadlines.  Contact Chris Barfield, Director of Athletics with questions.

  • By agreeing to the terms and conditions of enrollment, parents or guardians give permission for their child to be photographed or videoed for EDS and/or Hilton-Green publications or public relation videos, advertisements, online social media, and/or the EDS website unless a request otherwise is submitted in writing or via email to cbarfield@edscc.org.
