By entering my name below, as the applicant's parent or guardian, I am stating that I give my consent for the applicant to participate in the volunteer program with the North Clackamas Parks and Recreation District, and hereby release Clackamas County and NCPRD from any and all claims for damages, injuries or sickness that may occur while engaged as a volunteer in the program. I have also satisfied myself that the above listed minor is physically able to undertake and complete this activity.

I release, indemnify and hold harmless Clackamas County and North Clackamas Parks and Recreation District, its officers, agents and employees from any and all claims, actions and demands that may arise from my actions as a volunteer.
