D&M FIT LIABILITY WAIVER AND RELEASE AGREEMENT

I understand that participation in fitness activities, strength training, conditioning, and use of gym equipment involves inherent risks of injury.

By participating in activities at D&M FIT, I voluntarily assume all risks associated with physical exercise.

I certify that I am physically capable of participating in exercise activities and that I will notify D&M FIT of any medical conditions that may affect my ability to exercise safely.

I agree to follow all safety instructions provided by D&M FIT.

I release and hold harmless D&M FIT, its owners, trainers, and affiliates from any liability for injuries or damages resulting from participation in gym activities.

I understand that D&M FIT is not responsible for lost or stolen personal belongings.

I consent to emergency medical treatment if necessary.
