Liability Waiver and Transportation Agreement
This waiver pertains to the Day with Mary event scheduled for May 30, 2026, held at Villanova College, King City. This event is an initiative of the Archdiocese of Toronto, with school bus transportation provided by Blessed Trinity Parish. Please note that there is no charge for the event or the transportation.

The bus will depart from Blessed Trinity Parish at 9:00am and is expected to return at approximately 5:15pm. For those driving to Blessed Trinity Parish, vehicles may be left in the church parking lot for the duration of the event.

Important: One waiver must be completed for each adult requesting transportation.

I understand that this document pertains to the bus transportation provided by Roman Catholic Episcopal Corporation for the Diocese of Toronto, in Canada, operating name Blessed Trinity Parish (3220 Bayview Avenue, Toronto) for the 'Day with Mary' event ("Event") to Villanova College, King City, on May 30, 2026. I acknowledge that my registration for the event itself is separate from this form, and this agreement specifically governs the travel arrangements and provides a liability waiver for the duration of the event, which is an activity of the Archdiocese of Toronto.*

I understand the above statements and agree to the terms of this liability waiver and transportation agreement.
AGREEMENT AND WAIVER / RELEASE OF LIABILITY for the Roman Catholic Episcopal Corporation for the Diocese of Toronto, in Canada (Archdiocese of Toronto) including but not limited to Blessed Trinity Parish. In consideration for being allowed to participate in this Event, which I do freely and voluntarily for my own personal benefit, I hereby take action for myself, my executors, administrators, heirs, next of kin, successors and assigns and agree to the following statements:*

I understand that I am responsible for arriving at the designated pickup points on time for myself and any minors under my care. Failure to board the bus at the prescribed time, either at Blessed Trinity Parish or at the Event site, will result in me or the minor participant(s) being responsible for our own transportation arrangements.I understand that the school bus provided for this event is not accessible. I agree that I, and any minors under my care, will not bring walkers and/or strollers on the bus. I acknowledge that I, and any minors under my care, must be able to board and exit the vehicle independently.I assume full responsibility for any and all injuries, damages (both economic and non-economic), and losses of any type, which may occur to me , and I hereby fully and forever release and discharge the Archdiocese of Toronto, its insurers, employees, officers, directors, and associates, from any and all claims, demands, damages, rights of action, or causes of action, present or future, whether the same be known or unknown, anticipated, or unanticipated, resulting from or arising out the use of the transportation and facilities provided.I contract on behalf of myself, my heirs, executors, administrators, successors and assigns, that Archdiocese of Toronto or Blessed Trinity Parish and its insurers, employees, appointed instructors, including but not limited to its officers, directors, and associates, shall not be liable for any damages arising from personal injuries (including death) sustained by me in, on, or about the premises, the school bus, or as a result of participating in any Event activities and/or the use of the facilities, regardless of whether such injuries result, in whole or in part, from the negligence of the Archdiocese of Toronto or Blessed Trinity Parish.I understand that the Archdiocese of Toronto or Blessed Trinity Parish is not responsible for property that is lost, stolen, or damaged while in, on, or about the school bus or Event.I understand I am solely responsible for safety and wellbeing of myself and the minor participant(s) under my care.I acknowledge that every participant aged 18 or older must sign their own individual waiver. For any minor participant(s) under the age of 18 listed below, I confirm that I am their parent or legal guardian, and I hereby sign this waiver on their behalf, agreeing to be bound by all terms of this document.I release, hold harmless and forever discharge the Archdiocese of Toronto and/or Blessed Trinity Parish staff, appointed instructors, including but not limited to contractors, caregivers, volunteers, directors, employees and affiliates, from any and all liability, claims, losses, damages, costs or expenses, and waive any such claims against and such persons or organizations, arising directly or indirectly from, or attributable in connection with the event activities and programs, onsite or offsite through the sponsorship and organization of the Archdiocese of Toronto and/or Blessed Trinity Parish.
