WEEKLY SUMMER CAMPS PACKED WITH FUN!

This camp offers flexible weekly enrollment for up to six weeks. Each week is centered around an engaging theme, with 1-2 exciting field trips and plenty of time to explore and enjoy ULS’ beautiful campus alongside friends.

Sprinter Van Permission Slip
In the interest of providing students with educational and co-curricular opportunities outside the University Lake School (ULS) campus, transportation of students to activities will take place utilizing ULS-owned vehicles. This permission slip provides parent/guardian consent for the student to travel in a ULS vehicle.

The ULS vehicle may only be driven by approved ULS employees who have undergone a safe drivers background check through the school insurance company. The ULS employee also agrees they have a valid Wisconsin Drivers License, are fully insured, and have no major traffic violations or at-fault accidents on their driving record in the last 5 years. Students traveling alone in the vehicle with a ULS employee will be avoided.

For summer camps and experiences of 2026, I give permission for my child to be transported in a ULS-owned vehicle driven by a ULS employee.

I understand that this permission will only be used for travel to and from school-sanctioned activities that originate and end at University Lake School Campus. Parents/guardians wishing to take their children home following the day trip(s) may do so and agree to inform the staff member in advance.

In consideration for ULS transporting my child in the manner described herein, and on behalf of both my child and myself, I expressly release, waive, and discharge any and all claims, liabilities, allegations, rights, or causes of action whatsoever against for any losses, damages or injuries, whether incurred by my child or others, arising out of, concerning, or in any way connected with the transportation of my child as described herein, including but not limited to, personal injury, damage to property, and the rendering of emergency medical attention, treatment or procedures, and I agree to indemnify and hold harmless ULS, its officers, employees, and agents, for same. None of the above should be read to include any claims or causes of action for reckless or intentional conduct, which cannot be released, waived, or discharged under Wisconsin public policy.

As the legal parent or guardian of the above named child(ren), and in the event of an injury or illness which requires immediate examination or treatment, in the opinion of University Lake School officials, I authorize University Lake School on my behalf to have my child transported by car or ambulance to the nearest hospital. Necessary emergency treatment may be given by any doctor on call.

The parents of a student who incurs an injury or suffers an illness that requires more than first aid will be notified as soon as practical after the injury occurs. In an emergency, this notification may occur after the student has been transported to and/or treated by a doctor, clinic or hospital.

I understand that the school assumes no financial responsibility for medical care or ambulance conveyance. We further understand that ULS does not carry medical expense insurance for the benefit of any student who may be injured while participating in school activities and that ULS assumes no responsibility for such medical expense. We further understand that a student who does not have health insurance coverage will not be permitted to participate in athletics unless this statement is signed and filed with the Head of University Lake School. We consent to having our son/daughter participate under these conditions and authorize the treatment of injuries and illness incurred by our child in accordance with the procedure described above.
