In the interest of providing students with educational and co-curricular opportunities outside the University Lake School (ULS) campus, transportation of students to activities will take place utilizing ULS-owned vehicles. This permission slip provides parent/guardian consent for the student to travel in a ULS vehicle.
The ULS vehicle may only be driven by approved ULS employees who have undergone a safe drivers background check through the school insurance company. The ULS employee also agrees they have a valid Wisconsin Drivers License, are fully insured, and have no major traffic violations or at-fault accidents on their driving record in the last 5 years. Students traveling alone in the vehicle with a ULS employee will be avoided.
For summer camps and experiences of 2026, I give permission for my child to be transported in a ULS-owned vehicle driven by a ULS employee.
I understand that this permission will only be used for travel to and from school-sanctioned activities that originate and end at University Lake School Campus. Parents/guardians wishing to take their children home following the day trip(s) may do so and agree to inform the staff member in advance.
In consideration for ULS transporting my child in the manner described herein, and on behalf of both my child and myself, I expressly release, waive, and discharge any and all claims, liabilities, allegations, rights, or causes of action whatsoever against for any losses, damages or injuries, whether incurred by my child or others, arising out of, concerning, or in any way connected with the transportation of my child as described herein, including but not limited to, personal injury, damage to property, and the rendering of emergency medical attention, treatment or procedures, and I agree to indemnify and hold harmless ULS, its officers, employees, and agents, for same. None of the above should be read to include any claims or causes of action for reckless or intentional conduct, which cannot be released, waived, or discharged under Wisconsin public policy.
