In the file below (ideally Excel or CSV format), please make sure to include columns for the following fields:
- De-identified employee identifier (1,2,3,4)
- State
- Pay frequency (hourly, weekly, biweekly, etc)
- Gross pay per pay period
- # of W4 elections / Marital Status (if you don’t have the # of W4 elections, just put their federal marital status (Single, Married, etc))
