In the file below (ideally Excel or CSV format), please make sure to include columns for the following fields:
- De-identified employee identified (1,2,3,4)
- State
- Pay frequency (weekly, biweekly, monthly, etc)
- Gross pay per pay period
- # of W4 elections (if you don’t have the # of W4 elections, just put "1" or their pre-tax deductions)
