The following authorizations explain the repair process, payment responsibilities, and related terms. Please review carefully.

I authorize Alpha Dent Solutions ("Repair Facility") to inspect my vehicle and perform any necessary disassembly for the purpose of preparing an estimate and/or completing paintless dent repair (PDR). I understand that limited disassembly may be required to properly access damaged areas.

If I choose not to proceed with repairs after inspection and/or disassembly has occurred, I agree to pay a non-refundable inspection fee of $150 and/or a disassembly fee of $500, as applicable, due immediately.

I authorize Alpha Dent Solutions to perform paintless dent repair services as outlined in the written or electronic estimate, including any supplements approved by me and/or my insurance carrier. I understand that any quoted repair time or completion date is an estimate only and may change due to insurance delays, parts availability, weather events, or claim volume.

I understand and agree that I am personally responsible for payment of the full repair amount, including but not limited to:

• Insurance deductibles
• Any portion not covered by insurance

• Customer-pay repairs

Payment is due in full upon completion of repairs and prior to vehicle release, regardless of insurance payment status.

If repairs are covered by insurance, I authorize and direct my insurance company to issue payment directly to Alpha Dent Solutions. I grant Alpha Dent Solutions a limited power of attorney solely for the purpose of endorsing insurance checks related to repairs performed on my vehicle. This authorization automatically terminates once payment is received in full.

If the vehicle is not picked up within 48 hours after notification that repairs are complete, I agree to pay a storage fee of $65 per day, as permitted by Texas law.

To secure payment of all amounts owed, I grant Alpha Dent Solutions a mechanic's and/or statutory lien on the vehicle as allowed under Texas law. The vehicle may be retained until payment is made in full.

I acknowledge that certain services (including but not limited to detailing, glass, alignment, or mechanical work) may be sublet to qualified third parties when necessary to complete repairs.

I authorize Alpha Dent Solutions and its employees or subcontractors to operate my vehicle for purposes of testing, transporting, or completing repairs.

I agree to remove all personal belongings from the vehicle prior to service. Alpha Dent Solutions is not responsible for loss of or damage to personal items left in the vehicle.

Please review the warranty information below. Your initials confirm that you understand the warranty coverage and limitations.

Alpha Dent Solutions provides a limited lifetime warranty on paintless dent repair (PDR) workmanship only, for as long as the original customer owns the vehicle.

This warranty applies solely to dents repaired using the paintless dent repair process and guarantees that the repaired areas will not fail or return to their pre-repair condition due to defects in PDR workmanship.

This warranty is limited strictly to PDR workmanship and does not apply to any non-PDR services, components, or repairs, which are addressed separately below.

Alpha Dent Solutions may perform, arrange, or coordinate certain non-PDR or sublet services as part of the overall repair process, including but not limited to:

 

• Windshields and other glass
• Hoods, panels, and structural components
• Electrical components and wiring
• Power sunroofs and related mechanisms
• Body work, refinishing, and paint work
• Mechanical work, alignments, or calibrations

Alpha Dent Solutions provides a limited three (3) month warranty on such sublet or non-PDR services, beginning on the date repairs are completed, only to the extent that a defect is directly attributable to the workmanship of the specific non-PDR or sublet service performed.

Alpha Dent Solutions' responsibility under this section is limited to facilitating inspection, repair, or correction of the specific non-PDR or sublet service within the three (3) month period and does not extend beyond the scope of that service.

• Pre-existing damage, prior repairs, repainting, or refinishing
• Damage resulting from subsequent accidents, impacts, hail, misuse, or acts of nature
• Normal wear and tear, road hazards, or environmental exposure
• Failures unrelated to the specific PDR or non-PDR service performed

Under no circumstances shall any warranty provided by Alpha Dent Solutions be construed as a vehicle-wide warranty or expanded beyond the specific repair or service performed.

Except as expressly stated above, Alpha Dent Solutions disclaims all other warranties, express or implied, including any implied warranties of merchantability or fitness for a particular purpose, to the fullest extent permitted by Texas law.

I understand that:

 

• Alpha Dent Solutions provides a limited lifetime warranty only for paintless dent repair (PDR) workmanship
• A separate three (3) month warranty applies to non-PDR or sublet services
• No vehicle-wide warranty is provided

I authorize Alpha Dent Solutions to contact me by phone, email, or text regarding my repair, including automated or prerecorded messages. Consent is not required as a condition of purchase.

This agreement shall be governed by and interpreted under the laws of the State of Texas.

By signing below, you acknowledge and agree that:

• You are the owner of the vehicle or are authorized to act on behalf of the owner
• You have read and understand the terms of this Work Authorization & Customer Agreement
• You authorize Alpha Dent Solutions to perform the services described above
