Participation is voluntary and does not create an employment or contractor relationship unless explicitly stated in writing.

Coaches acknowledge inherent risks associated with working with youth and assume responsibility for those risks.

To the fullest extent permitted by Arizona law, the Coach agrees to release and hold harmless Edge High School claims arising from participation, except in cases of gross negligence or willful misconduct.

Edge High School does not provide personal medical or liability insurance for volunteers.

Participation is contingent upon fingerprint clearance as required by Arizona law.

The Coach acknowledges that, pursuant to Arizona Revised Statutes § 13-3620, individuals who work with, supervise, or have responsibility for minors are designated mandatory reporters under Arizona law. The Coach understands and agrees that, as a condition of participation in the Edge High School Success Coaching Program, the Coach must comply with all applicable mandatory reporting laws and Edge High School Mandatory Reporting Guidelines, as provided in the separate Mandatory Reporting document.

All student information is confidential pursuant to FERPA and school policy.

Edge High School may terminate volunteer any time for safety or policy violations.

IN WITNESS WHEREOF, the undersigned acknowledge that they have read, understand, and agree to the terms and conditions of this Legal, Insurance, and Hold Harmless Agreement and voluntarily execute this Agreement as of the dates written below.
