  • Kwe/hello and thank you for taking the time to consider completing We Matter’s survey on Polysubstance Use in Indigenous Communities, specifically 2SLGBTQIA+ communities. 

    We Matter has gotten the exciting opportunity to work with the Public Health Agency of Canada to build out a toolkit for understanding and supporting polysubstance users, through our own Indigenous voices. This is created by us and for us to use within our communities and networks to uplift ourselves and other Indigenous youth.

    Please note, jotform automatically collects emails so your email will be displayed to us. Our commitment in keeping this survey anonymous is that only the above email (Alexia, Managing Director) will see emails. Prior to analyzing survey results we will remove emails to ensure anonymity.

    We will not keep this data longer than the project end date, March 31st 2027. All surveys will be destroyed at that time or earlier. Please do not include your name or email elsewhere in this survey.
