2026-2027 Children and Youth Church Registration

For any child or youth (birth-12th grade) who attends any Kingswood event, class, camp, or meeting. All children and youth must be registered each new school year to ensure our records are accurate and up-to-date. Each family will only need to complete one registration form containing information about all children/youth in the family.

My child(ren)/youth have permission for photographs to appear on social media and/or public communications. (no full names will ever appear publicly)
My child(ren)/youth have permission to leave the Kingswood campuses during events within walking distance. (This does not include any transportation as it will be within walking distance from the church and will always be communicated when and where children/youth go with the appropriate adult supervision. Any events requiring transportation will include a specific permission for that event.)
In the event of an emergency or non-emergency situation in which medical treatment is required as a result of participation in the Kingswood United Methodist Church outings or activities listed below, every reasonable effort will be made to contact the persons listed as caregivers. If unsuccessful in contacting the persons listed, consent/permission is given for treatment by competent medical personnel.

Further, consent/permission is hereby given to the Director of Children, Families, and Welcoming, the Coordinator of Youth Ministries, Pastors, and other Kingswood United Methodist Church leadership to hospitalize, secure proper treatment for, and to order injection, anesthesia, or surgery (under recommendation of qualified medical personnel). I also agree that my insurance company will be used for such medical care, and I am aware that I may be billed by the medical provider for any medical treatment not covered by my insurance.
