RENTAL EQUIPMENT COVERAGE

1. Commercial General Liability Coverage ($1,000,000 per Occurrence, $2,000.00 per Aggregate)
2. Equipment Property coverage for replacement value of the equipment, with a $25,000 minimum. Mountain Valley Power Rentals & Sales LLC must be named as Loss Payee and the maximum deductible is $15,000.
3. Auto Liability coverage ($1,000,000), including hired and non-owned automobiles (for any equipment that is licensed for road use)
4. Workers Compensation Insurance (if required by law, with a minimum of $500,000 in Employer's Liability coverage)
5. Mountain Valley Power Rentals & Sales LLC must be named as Certificate Holder and Additional Insured.
6. All policies must include a waiver for subrogation.
7. 30-DFay cancellation notice.
