Membership Incentive MY27 T-Shirt Selection

All members are encouraged to complete this form.
Renewal must take place between April 1-30 to earn free t-shirt. Council will verify all renewals prior to t-shirts being mailed.
Troop co-leaders are welcome to place t-shirt orders for girls in their troop. A separate form is required for each Girl Scout.
All t-shirt selections are final at midnight May 10. No exchanges will be given. If more than one form is submitted per member, council will use the most current submission for t-shirt size.
All Spring Membership incentives will be mailed to Troop Co-Leaders or Juliette Girl Scouts in August.
