COMMUNITY SERVICE AWARD NOMINATION FORM
Each year Austin's Commission on Aging recognizes people making exceptional contributions to older adults in the City.

There are two categories to choose from: 1) Volunteers, and 2) Paid professionals. An individual cannot be nominated in both categories.

Please fill out all the information on the form and help us acknowledge the outstanding efforts of people benefitting Austin's older adult community.

Awards will be announced at the May 13th meeting of the Commission on Aging, at City Hall.
