It is our policy to contact the parent or guardian as soon as possible in the event of a serious accident or injury.  If the information has changed on the day of registration please see the health officer at that time.  Please bring all medication in ORIGINAL CONTAINER or you will not be allowed to leave the medication or vitamins at the camp.  No exceptions.  All prescriptions must be in that campers name and the correct dosage.

Complimentary Transportation Services:

Through the great partnership with Transformation Health, LLC, Transformation Nation, Inc. will be providing complimentary transportation services for all children enrolled in PRP Services. This transportation will be provided by a third-party transportation service, Double J Transportation, LLC.

Please understand that all parents will be required to complete our Transportation Service consent and waiver form to participate in the transportation services. Parents will consent to the third-party transportation service for all filed trips, daily pick up and drop off services, and other trips that may be scheduled or for non-scheduled events.

Please be aware that some Healies may receive transportation by our previously approved Transformation Health/Nation staff. These staff members have been cleared to transport children with driving records and current driver’s license on file.

When children are being picked up from Transportation, we asked that all Healies be ready at the door for pickup. We want to make sure that all Healies are dropped off in an appropriate time frame.

• Healies will be dropped off at the home office:

              *All Healies will be delivered and checked off by the Transportation                             Driver and check in by Transformation Nation/Health Staff.

              *Healies will then be escorted to the Healing Suites by staff.                

Field Trips:

Healies are required to wear their camp t-shirts and bringing their camp back sacks on field trips. This is required so that each child is identifiable from their shirts and the back sacks are for transporting any personal items they may need. (i.e. Rx)

Healies are also required to bring any spending money they desire to bring not to accede $25. We do not want Healies to have access of spending money to protect them from loss of funds.

      * Camp staff and or their parent volunteers are NOT RESPONSIBLE for                        Healies personal belongings and or monies. 

 

All Healies parent/gaurdian are required to complete the below waiver agreement:

If your child (children) were referred by DSS (Department of Social Services) please click the lin below. 

Neither referral sources need to click the boxes for each week. Your children will be included in the full two month therapeutic summer camp! 

If paying with PayPal you will be redirected to PayPal once you hit submit.  You can either pay with credit card, or your personal PayPal account.  There is a $1.50 processing fee for using this option.  The remaining balance is due anytime before their week of camp.  If you have any issues or questions feel free to contact Cyrus at 443-873-9106.

If paying with a check, please send the registration fee as soon as possible.  You can pay multiple registration fees with one check.  In memo, please put the camper(s) name.  The remaing balance is due anytime before their week of camp.

By hitting submit and putting your initials, you agree that you are the parent or legal guardian of the above named healie, and are over the age of 18.  In case of medical emergency or general medical care, I give consent for medical teatment for the aboved named healie by authorized personnel. I understand that the above named healie will only be released to the names listed above, an update may be done at registration or in person. Any addional names must be submitted in writing or in person by the leagal guardian.  I certify that my child has my permission to attend camp and participate in all activites.  I authorize The Healing Suites of Transformation Nation, Inc. to use my healies's picture, testimony, and video in any promotional material (web,print,or media) My child may recieve all e-mails from the camp.
