2026 - 2027 UJS Registration Form
School Hours are Sunday 9:30am - 12:00pm unless otherwise specified. All students, including Mitzvah Core and Madrichim, must be registered.

School Programs & Fees
Gan Katan: 2 & 3 year-olds (with parents from 11:00 - 12:00) $175

Pre-Kindergarten - 2nd Grade: $400

3rd - 6th Grade: (Weekly Supplemental Hebrew as scheduled) $680

7th - 8th Grade: $400

Mitzvah Core: 9th-12th grade, 12:30 - 1:30pm (with lunch), cost TBD

Madrichim: Must be 14 or older, 8:45am - 12pm, $0

** ALL families will be assessed a $50 fee to help supplement the cost of security.

  FIRST METHOD: Use the "Pay tuition" button on UJS website: https://www.unitedjewishschool.org/registration-payment/
  SECOND METHOD: If you are paying by check: ALL Checks should be made payable to "Jewish Federation of Grand Rapids" and mailed or delivered to UJS.

VACCINATION AND OTHER MEDICAL INFORMATION

The routine vaccination of all students is an important public health matter. Both the Reform and Conservative movements believe that childhood vaccination is a mitzvah, based on the Torah's exhortation to "take utmost care and watch yourselves scrupulously" (Deuteronomy 4:9).

The requirement of all students at the United Jewish School is to be adequately immunized against all of the preventable childhood diseases as recommended by the American Academy of Pediatrics and the CDC's Advisory Committee on Immunization Practices; therefore, it is United Jewish School policy that age-appropriate compliance of vaccination is enforced, and to exclude individuals who are out of compliance. We require appropriate documentation of immunization status prior to the start of school.

Parents must provide vaccination records or documentation of medical exemptions from the state or their pediatrician prior to the start of school. Your doctor or your local Health Department can print an Official State of Michigan Immunization Record for you. You can also request forms here.

We strongly encourage parent/guardian involvement at UJS! Volunteering at your child’s school helps us build community, and allows families to foster individual connections as well. Permissions & Agreements
I grant permission for my child/children named above to be photographed during Beit Sefer B’Yahad/United Jewish School Activities. I understand that when photographs are used in print or online, no identifying information will be included.
I grant permission for my child/children named above to leave Beit Sefer B’Yahad/United Jewish School premises to participate in religious school field trips (the school will inform you about off campus trips and tell you about travel arrangements prior to the field trip).
I grant permission for UJS staff to call for emergency transportation to bring my child/children named above to an emergency room in case of emergency. I understand that every effort will be made to reach either or both parents by phone as soon as possible.
