  • Internet Policy, Waiver, and Consent Form for Internet Use by Minors

    The content of our program makes the use of laptops, desktops, or other devices and technology that can access the internet unavoidable. While we take pride in taking reasonable precautionary measures to prevent unauthorized access to the internet by minors, it is impossible for Aspire Coding Academy (“Company”) to enforce compliance with its user policy and rules at all times.

    Accordingly, parents must recognize that minors will be required to make independent decisions and use good judgment in their use of the Internet. Parents must participate in the education of their children regarding access to the Internet and must communicate their own expectations to their children regarding its use.

    I, {parentOrGuardianName} (parent), as a parent/guardian of {studentName} (minor), accept the following conditions.

    1. I understand that any Internet access during our class sessions is designed for educational purposes only. Access to any content on the internet that is not specifically directed by the instructor is not allowed.
    2. Because I recognize that it is impossible for Aspire Coding Academy to restrict access to all objectionable material, I will not hold Aspire Coding Academy responsible for materials acquired or contacts made on the internet with Company devices or during class sessions.
    3. I understand that a variety of inappropriate and offensive materials are available over the Internet and that it may be possible for my child to access these materials if he/she chooses. I also understand that it is possible for undesirable individuals, with bad intentions, to communicate with my child over the Internet, and that my child must take responsibility to avoid such communications if they are initiated.
    4. While I authorize the staff to monitor any communications to/from my child on the Internet, I recognize that it is not possible for the Company to monitor all such communications.
    5. I have made the decision that the benefits to my child of having access to the Internet outweigh the potential risks.
    6. I understand that any conduct that is in conflict with these responsibilities is inappropriate, and such behavior may result in consequences that may include but are not limited to the termination of Internet network services during class time or nonrefundable expulsion from the program.
    7. I have reviewed these responsibilities with my child, and I hereby grant permission to the Company to provide devices or technology that may have Internet network access or access the Internet during class.
    8. I understand that the Company, its directors, officers, employees, agents, and the Company’s successors or assigns are not liable for any damages or losses which may occur as a result of my minor child’s use of the Internet. I therefore release and hold harmless the Company, its directors, officers, employees, agents, and the Company’s successors or assigns, from any liability whatsoever that could arise as a result of my minor child’s use of the Internet.
    9. I further agree to compensate the Company for any expenses or costs incurred by the Company as a result of my minor child’s violation of this Policy.
    10. I have read the above with my child and s/he agrees to abide by the terms herein.
  • General Release and Waiver of Liability Form

    PLEASE READ CAREFULLY! THIS IS A LEGAL DOCUMENT THAT AFFECTS YOUR LEGAL RIGHTS!

    This Release and Waiver of Liability (the “Release”) executed on this {date} by {studentName} , a minor child (the “Participant” or “Student”), and {parentOrGuardianName}, the parent having legal custody and/or the legal guardian of the Participant (the “Guardian”), releases Aspire Coding Academy (the “Company”), a limited liability company organized and existing under the laws of the state of Texas and shall include in its definition the entity, each of its managers, directors, officers, employees, agents, successors, and assigns.

    Permission: I, the parent or guardian of the above-named child, do hereby give my consent to his/her participation in all activities of Aspire Coding Academy.

    Rules, Terms, & Conditions: Payment is due at time of registration and will not be refunded until proper cancellation notice is given. To give cancellation notice, the parent/guardian must provide written notice via email to contact@aspirecodingacademy.com at least 30 days prior to the start of the class. Students cannot submit cancellation notices. Aspire Coding Academy is committed to fostering a safe, inclusive, and respectful learning environment. Treat instructors, staff, and peers with respect. Any intentional damage, loss, or misuse of Academy-provided technology may result in financial liability for repair or replacement. Violations of this policy may result in disciplinary action, including warnings, removal from class, or dismissal from the program without a refund

    Waiver and Release: We, both the Participant and the Guardian, release and forever discharge and hold harmless the Company from any and all liability, claims, and demands of any nature, either in law or in equity, which arise or may hereafter arise from the activities as a Participant with the Company.

    Discharge of Liability: We understand and acknowledge that this Release discharges the Company from any liability or claim that we may have against the Company with respect to bodily injury, personal injury, illness, death, or property damage that may result from the services the Company provides or occurring while the Participant is receiving services.

    Insurance: I affirm that the Participant is covered by primary medical insurance and understand that I am responsible for my child’s medical bills if injury occurs. Further, we understand that the Company does not assume any responsibility for or obligation to provide me with financial or other assistance, including but not limited to medical, health, or disability benefits or insurance of any nature in the event of my injury, illness, death, or damage to my property. I expressly waive any such claim for compensation or liability on the part of the Company. I do fully understand and agree that the Guardian is responsible for his/her own child’s insurance coverage in the event of personal injury or illness as a result of participation in any activities of Aspire Coding Academy. I give my consent for medical treatment of my child/ward by the closest hospital, doctor, or medical facility should injury occur during a session.

    Assumption of Risk: As a Participant, I hereby expressly assume the risk of injury or harm to me from these activities and release the Company from all liability for injury, illness, death, or property damage resulting from the services I receive as a Participant. As the Guardian, I hereby expressly acknowledge and agree with this assumption of risk.

    Photographic Release: I grant and convey to the Company all right, title, and interests in any and all photographs, images, video, or audio recordings of me or my likeness or voice (or my child) made by the Company in connection with my (or my child/ward) participating in Aspire Coding Academy events.

    Internet Policy and Waiver: I have read and have signed the Company’s internet policy and waiver.

Students cannot submit cancellation notices. Students must be dropped off and picked up according to class hours. A 10-minute grace period is allowed.
