Thank you being a Foster Parent for an Adopt a Pet (AAP) dog. We appreciate your interest in giving a dog a loving home environment while they wait for their furever home.

It’s always best for all parties to have a clear understanding of expectations to avoid any confusion - we all want what is best for the dogs. This Acknowledgement Form is intended to clarify Adopt a Pet’s expectations of the Foster Parent.

Please initial to indicate you understand and accept the following:
The dog is owned by AAP. This means that AAP has the final say on what happens with the dog, including adoption decisions or the need to return the dog to the shelter.

I agree to keep the dog warm, safe, dry, and fed well.

I will treat the dog with kindness, love, patience and consistency.

I am responsible for bringing the dog to meet and greet appointments. If unable to do so at a specific time, I will make arrangements with the adoption team to get the dog to AAP prior to the meet.

I am responsible for notifying the Foster Coordinator if my fenced yard becomes damaged to the point it is no longer safe (AAP will temporarily house the dog if the fence is damaged to the point of being unsafe and the foster has scheduled repair.)

I agree there is no smoking allowed inside the house.

I agree to not let the dog off leash in any public places (dog parks, when walking along roads, etc), off leash ONLY allowed in fenced yards/areas.

No aversive training tools or techniques are to be used on the foster dog.

Any aggressive behavior must immediately be reported to the Kennel Manager or Foster Coordinator.

I agree to provide an email address to the Foster Coordinator as a form of communication, and will respond to any emails or phone calls within 24 hours.

I agree to keep the dog at home for at least the first week while it decompresses (i.e. no taking to friends house, out in public, etc).

If the dog becomes ill I will notify the Kennel Manager in a timely manner ((1) If an emergency situation, Kennel Manager is to be notified immediately. (2) If the dog is exhibiting an illness that does not appear to be life threatening, the Kennel Manager will be notified within 24 hours.

I agree to bring the dog to the AAP facility for any needed vaccinations, nail trims or other assessments performed at the shelter, or to be transported from the shelter to a veterinary appointment.

When taking a vacation, I agree to provide AAP with two weeks advance notice prior to the start in order to make alternative arrangements for my foster to be housed.

The dog will have access to chews and toys provided by AAP; any consumable, non-rawhide chew (beef cheeks, collagen rolls) will require supervision. The dog will not be given Greenies or rawhide.
