General Program Activities Authorisation Form
The program outlines the activities and theme of what is happening during that day at Vacation Care, however we have time throughout the day to have spontaneous activities and unplanned activities including programs and workshops onsite, activities suggested by children, free play, and playing outdoors in nature. This authorisation form allows your child to participate in these activities each day during Vacation Care.

I acknowledge that spontaneous activities may be provided and hereby give permission for my child to join in.
I acknowledge that my child can participate in activities in OOSH including: water play, mud play, gardening, swings, tyre swings and tree climbing.
Other onsite programs include Easter Tuesday with Buster the Bunny, Pottery Day, Tumbletown, Gaming Trailer, Junior Architects Workshop, Mini Golf, Earth Rocks Day, Mosaic Coasters, and Go Karts & Remote Control Car Racing.
I acknowledge children are not allowed to bring mobile phones or technology to Vacation Care unless stated on the program.
I acknowledge that children attending Vacation Care can bring their own wheels e.g., bike and/or scooter (NO wheelie shoes or skateboards). A helmet must be worn while riding.
I acknowledge that EHOOSH is not liable for damages, breakage, or loss of personal belongings, including toys, wheels, equipment, or devices.

Kicks Tenpin & Arcade Excursion Authorisation Form - Tuesday 14th April
The excursion will be held at Kicks Tenpin & Arcade in Parramatta Leagues Club. Children will be away from the service from 9:00am and return by 1:45pm. The ratio for this excursion is one educator to eight children and the anticipated number for this excursion is 60 children with nine educators. Children will be supervised by EHOOSH educators at all times. A risk assessment has been completed and is available at the service upon request. If you have booked your child for this excursion, please fill in the details below before moving on.

I give permission for my child to attend the excursion to Kicks Tenpin & Arcade in Parramatta Leagues Club on Tuesday 14th April 2026.
I acknowledge that Glenorie coaches private bus might not be fitted with seat belts.
Arrive by 9:00am, the bus will not wait for late families as we have a scheduled booking.
I acknowledge that my child is required to wear appropriate clothing, including enclosed shoes and socks. Additionally, I will ensure to pack a small backpack containing a hat, a drink bottle, and meals.
I acknowledge my child will be away from the service from 9:00am and return by 1:45pm. Lunch will be consumed at the site.
I acknowledge that if my child arrives at the service without an EHOOSH t-shirt and hat they will be given a new one and my account will be charged.
I hereby give permission for my child to participate in programmed activities and attend the excursion to Kicks Tenpin & Arcade in Parramatta Leagues Club as nominated on the program on Tuesday 14th April 2026, as well as participate in all walking required to attend the excursion of the premises of the service.

Top Ryde Movies Excursion (Super Mario Galaxy) Authorisation Form - Thursday 16th April
The excursion will be held at Top Ryde Event Cinemas. Children will be away from the service from 9:30am and return by 1:15pm. The ratio for this excursion is one educator to eight children and the anticipated number for this excursion is 60 children with nine educators. Children will be supervised by EHOOSH educators at all times. A risk assessment has been completed and is available at the service upon request. If you have booked your child for this excursion, please fill in the details below before moving on.

EHOOSH has hired a private cinema for viewing, the movie is 'Super Mario Galaxy' and is rated PG.
