The following is a basic statement of agreement and release of liability in granting access to the website files and pages of your website that were created by Campaign Agents. Please read the following, fill out the fields below and then submit this form by clicking the submit button. Upon receipt of your form submission, the WordPress login user name, password and login link will be emailed to the authorized email specified above. Thank you.
STATEMENT OF AGREEMENT AND RELEASE OF LIABILITY:
Typically access to a client's website and it's web pages created by Campaign Agents, is requested for the purpose of having another website designer, programmer, web developer, website agency, or any person(s) besides the original web designer, including the client, make changes to the website pages through the WordPress dashboard.
Whether or not this is the case, upon providing access, the authorized person named below agrees that Campaign Agents and it's designer(s), affiliate(s), owner(s) and agent(s) are released of any liability, contractually, financially or otherwise, for any errors, mistakes, deletion of or damage to, any of the website files/pages (accidental or intentional) by any person(s) from “your company” or otherwise affiliated to or hired by “your company” from accessing such website files/pages, whether or not adds, moves, changes, or updates were made or not. Therefore, you or your company shall make no claims against Campaign Agents or it's designer(s), affiliate(s), owner(s) and agent(s) for any website related issues or errors following the submission of this form.
Third-party Applications & Integrations:
Upon being granted access to any third-party or connected systems, including but not limited to automation platforms (such as Zapier, Make and/or N8N), form platforms (such as Ninja Forms and/or Jotforms, etc.), Google applications or any other integrated services, the client assumes responsibility for any changes made by themselves or any third party. Campaign Agents and it's designer(s), affiliate(s), owner(s) and agent(s) are released of any liability, contractually, financially or otherwise, for any errors, mistakes, deletion of or damage to any part(s) of any of these integrated systems resulting from such changes. Campaign Agents and it's affiliates remain responsible only for work it directly performs that has been authorized by the client through an approved quote, work order, or existing service terms.
Further, should damage, deletion or otherwise any errors occur to the website files, pages or any part of the website or external integrations, inclusing but not limited to web forms, automations, Google Sheet scripts - due to other person(s) accessing the website files/pages and a request is made to Campaign Agents to perform any website restoration, repair, rebuild or backup installation that may be required, all such labor will be subject to normal billing rates at that time. All release of website access credentials shall be in accordance with Campaign Agents terms and policies.
This release applies only to actions performed by parties other than Campaign Agents and it's affiliates. Campaign Agents and it's affiliates remain responsible for work it directly performs under a separate written service agreement.
