Please do not upload documents here unless you have already submitted your child's application.

Applications are not considered complete and will not be reviewed without the required documentation:
- Child's birth certificate
- Child's immunization record
- Child's medical form from the 4-year-old well child visit (can be submitted upon acceptance)
- Income verification. Ex: paystubs (1 month), W2, unemployment documented on application, etc.
- If not parent, documentation of legal custody/guardianship
