PART A: Assumption of Risk

I hereby request that my child, named above, be admitted to Precision Player Development Inc. (PPD Inc.) activities. In the event of an emergency, I hereby authorize the Director(s) and Instructor(s) of Precision Player Development Inc. (PPD Inc.) to act for me in their best judgment to provide or arrange first aid as deemed neccessary. I understand that I am responsible for all costs associated with medical care.

PART B: Release of Liability

In consideration of being allowed to participate in any way in the Precision Player Development Inc. (PPD Inc.) Program, related events and activities, the undersigned acknowledges, appreciates, and agrees that:

1. The risk of injury from the activities involved in this program is significant, and while particular rules, equipment and personal discipline may reduce this risk of serious injury, it does exist; and,

2. I KNOWLING AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation and,

3. I willingly agree to comply with the stated and customary terms and conditions for participation. If however I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest trainer immediately; and,

 4. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Precision Player Development Inc. (PPD Inc.), Their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises (Centennial Arena & BMO Soccer Field) used to conduct the event (.Release's.), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE.

I have read this release of liability and assumption of risk agreement, fully understand its terms, understand that I have given up substantial rights by signing freely and voluntarily without any inducement.

PART C: Media Release

Photos and video could be posted on Precision Player Development Inc. (PPD Inc.) web page, social media pages or used in Precision Player Development Inc. (PPD Inc.) publications or promotional materials

The purpose of the marketing, photo, print and/or video to be gathered is to increase awareness of Precision Player Development Inc. (PPD Inc.) and the value of our services.
