I understand that my child/children may participate in physical activities such as those held during Game Time. As with any physical activity, there is a risk of injury. I fully accept this risk and hold Sweet Springs Baptist Church and its volunteers harmless from any and all liability action causes of action claims, expenses, and damages on account of injury to my child, property, even injury resulting in death which I now have or which may arise in the future in connection with the activity or participation in any other associated activities.

In the event of an emergency that requires medical treatment for the child named above, I understand that every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give permission to the Camp volunteers to contact emergency services to provide the care necessary for my child's well-being. I assume responsibility for all costs connected to any accident or treatment of my child.

Sweet Springs Camp Rules
ALL youth must sign indicating their agreement to these rules. NO ONE is allowed to attend the camp without a signed parental permission slip and contact number(s).
Each camper is expected to stay the ENTIRE TIME.
Camp participants will not be allowed to go outside of the campgrounds without adult supervision, and if this rule is broken the parent will be called and expected to pick up their student regardless of the hour.
NO personal video games, trading cards, or electronic devices-phones should be brought to kids camp. Teens your electronic devices MUST stay in your cabins at all times.
Any possessions or bags in question must be surrendered at the entrance to the camp. Special medication or required items should be indicated on the parent permission form.
No illegal drugs, alcohol, cigarettes/electronic cigarettes, vapes, or dangerous materials.
ALL camp participants must respects others' physical boundaries. Inappropriate behaviour (physical intimacy or sexual harassment) is not permitted. ALL youth must repect the NO PDA (Public display of affection) rule. KEEP YOUR HANDS TO YOURSELF.
It is ecpected that camp participants will participate in the program.
Be considerate and respectful of other participants and leaders.
Be respectful of the church facilities and clean up after yourself.
Dress appropriately. SHORTS MUST BE PAST YOUR KNEECAPS. If they're not, wear pants. No tank tops. You MUST wear dark-colored clothing in order to articipate in water games.
Speak appropriately. We do not tolerate foul or inappropriate language.
Stealing and Vandalism will not be tolerated.
If there are youth that are able to drive, it is highly recommended that they drive only themselves and their siblings. No friends are allowed to leave the camp with a friend unless parents are aware.
Sleeping areas will be strictly enforced. (Boys area/Girls area) Boys stay on the boy's side. Girls stay on the girl's side. NO BOYS in girl cabins, and NO GIRLS in boy cabins.
Violation of these rules will result in disciplinary action and may includee the participant being asked to leave the camp. Parents will be called to pick them up.
