I, the undersigned parent or legal guardian of the above-named minor, give permission for my child to participate in activities and use the equipment at Hart-Cam Fitness Centre. I understand the facility is intended for individuals 13 years of age and older and that youth members participate at their own risk.
Assumption of Risk
I acknowledge that physical exercise and the use of gym equipment involve inherent risks including but not limited to muscle strains, sprains, slips, falls, equipment-related injuries, or other unforeseen injuries that may occur during participation.
Release of Liability
On behalf of myself and the minor listed above, I hereby release and discharge Hart-Cam Fitness Centre, the Municipality, its staff, volunteers, and representatives from any liability, claims, damages, or causes of action arising from participation in gym activities or use of equipment.
Code of Conduct
The participant(s) has read the provided Code of Conduct and agree to follow all gym rules, use equipment properly, treat staff and other users with respect, clean equipment after use, and report damaged equipment or injuries. Failure to follow facility rules may result in suspension or cancellation of membership.
Medical Acknowledgement
