This Liability Waiver and Release of Liability is executed by the undersigned participant or volunteer in favor of the Korey Johnson Foundation, Heroes Care, Leap Of Faith Church and Faith Miracle Temple, including their directors, officers, staff, volunteers, partners, and representatives.

I understand that I am voluntarily participating in the Community Clothing Giveaway event hosted by the Korey Johnson Foundation, Heroes Care, and Faith Miracle Temple. I understand that my participation is completely voluntary. {signature31}

I understand that participation in a community event may involve certain risks, including but not limited to slips, falls, lifting items, interactions with others, weather conditions, or other unforeseen incidents. I knowingly and freely assume all such risks, both known and unknown. {signature31}

In consideration of being permitted to participate in this event and receive or assist with free items, I hereby release, waive, and discharge the Korey Johnson Foundation, Heroes Care, and Faith Miracle Temple, along with their directors, officers, staff, volunteers, agents, partners, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, injury, or illness that may occur as a result of my participation in this event. {signature31}

I understand that all clothing and items provided at this event are free of charge and distributed "as is." The hosting organizations make no guarantees regarding size, condition, or suitability of any items received. {signature31}

I agree to exercise caution and personal responsibility while attending the event. If I choose to lift, carry, or transport items, I do so at my own risk. {signature31}

I agree to indemnify and hold harmless the Korey Johnson Foundation, Heroes Care, and Faith Miracle Temple from any claims, damages, losses, or expenses resulting from my actions during participation in the event. {signature31}

I grant permission for photographs or video taken during the event to be used by the Korey Johnson Foundation, Heroes Care, and Faith Miracle Temple for promotional or community awareness purposes. {signature31}

By signing below, I acknowledge that I have read this waiver, fully understand its terms, and sign it freely and voluntarily.
