    Step 1: Submit Tournament details using the fields below on this FORM

    Step 2: Upload the Required documents as follows and using the Upload Field on this form:

    • Upload Tournament or Jamboree Rules in PDF or Word format for the registrar to confirm (must follow checklist)
    • Upload Certificate of Insurance (for Non-CAHA Members or hosts)
    • Download the Fillable USAH Tournament form

    Complete the fillable USAH Tournament form, save it as a PDF, and upload it below.

     

    Step 3: Tournament Fees

    $250 payable to USAH
    $50 payable to RMD
    $50 payable to CAHA (if hosting full-ice Girls, Youth, or HS tournament)

    *No CAHA Fee is required for the following events: 

    • Pond, Adult
    • Adaptive
    • 1/2 ice or Cross-ice 8U Jamboree Tournaments

     

    All fees must be paid via Check and mailed to the Colorado Registrar.

    If hosting multiple tournaments, you can combine all USAH fees on one check, all RMD fees on one check, and all applicable CAHA fees on one check. You must still submit this form for each tournament.

  • As the tournament director or on behalf of the tournament director, I attest that:

     

    • The USAH Sanctioned Tournament Safe Sport Protocol has been read and will be followed in the operation of this tournament or jamboree. 
    • We will post or make our tournament rules available to all participants.
    • We will adhere to all our tournament rules.
    • We will check USAH 1-T rosters PRIOR to any tournament games for ALL participating teams. In the case of any tournaments/jamborees allowing participants to register as individuals 1-T rosters or proof of individual USAH registration will be checked prior to tournament games as will proof of SafeSport completion for 2009 and older players and required coaching credentials for all participating coaches.  We understand that failure to do so could lead to fines or other disciplinary consequences.
    • We will collect and submit travel permits from any Canadian teams to the Colorado Registrar at least 14 days prior to the start of the tournament.
    • We will submit an International Inbound Competition Form to the Colorado Registrar at least 30 days in advance of the tournament in order for any non-US/non-Canadian team to participate.  Changed to Colorado 
    • We understand that the tournament will need to pay an additional fee to USAH in order for an international (non-Canadian) team to participate. 
    • We agree to pay $12.50 per game to cover scoring costs once invoiced. Only applies to full ice youth, girls, or HS tournaments. Capped at 100 games per tournament for CAHA member hosts.
