Village Tots Ongoing Payment Form

This form is for families whose child has already attended Village Tots and completed the enrollment form. You may use this form to submit weekly payments or maintain a monthly membership for continued participation in classes. If your child has not yet attended Village Tots, please complete the Village Tots Enrollment Form first, which includes registration and initial payment. Once your child is enrolled, you may use this form for future weekly or monthly payments.
