Guam Basketball Confedeation Liability Waiver & Release Form



I,   First Name   Last Name   acknowledge that participation in basketball activities involves inherent risks, including but not limited to : Collisions with other players, falls and physical contact, heat-related illness, and accidental injury caused by facilities, equipment, or playing conditions. I understand these risks cannot be fully eliminated, even with safety measures in place.

Assumption of Risk
I allow my child to participate in this basketball event and assume all risks of injury, illness, or harm, whether know or unknown, arising from participation.

In the event of an emergency, I authorize the program organizers, coaches, and staff to :

1. Provide First Aid
2. Contact Emergency Services

Release of Liability
In consideration of participation, I hereby release, waive, and hold harmless :

Guam Sports Academy, its coaches, staff, volunteers, sponsors, facility owners, and agents

from any and all liability for injuries, accidents, damages, or losses arising from participation in this basketball event, except in cases of proven gross negligence or intentional misconduct.

I agree to not pursue legal action for any claims related to ordinary negligence.

Photo & Video Release
I grant permission for photos/video of the athlete to be used for program promotional content.

Participant & Parent/Guardian Agreement
I certify that:

• The participant is physically capable of participating in basketball activities.
• I have read and understand this waiver.
• I am signing voluntarily and understand its legal consequences.
