Fever Medication Consent Form (100°F and Above)
I give permission for staff at Toddler Town Daycare to administer a non-prescription fever-reducing medication (Children’s Motrin) to my child in the event their temperature reaches or exceeds 100°F, until I arrive at the center.
I understand and agree to the following:
Fever-reducing medication will only be administered when absolutely necessary, in the event of a sudden increase in my child’s temperature. It will not be given on a routine or scheduled basis.
This authorization is intended solely to temporarily reduce my child’s fever and help prevent escalation that could require emergency medical attention, until I arrive.
I understand that for staff to administer any other non-prescription medication, I must provide written authorization from a physician, dentist, or nurse practitioner.
I understand that for staff to administer prescription medication, I must:
Complete all required Medication Consent Forms included in the enrollment packet, and Provide the medication in its original container, clearly labeled with:
Child’s name
Date prescribed
Dosage instructions
I understand that I am required to pick up my child within one (1) hour of being notified of a fever.
I understand that my child may not return to the center until they are fever-free for at least 24 hours without the use of fever-reducing medication.
