Participant Commitment

I understand by completing this form my child will be participating in the 2026 ADC Student Show at Orange Coast College on June 6,2026. I further understand this annual showcase includes Dress Rehearsal Week and a Stage Performance at the Show which are mandatory events if agreeing to participate.

I understand that an excessive amount of absences leading up to the show without communication may result in removal from the dance.
Costume

I understand I will be responsible to bring my child to the front desk within 7 days of completing this form to get them measured for her/his costume. All costume measurements with our front desk staff should be completed by March 30th. If your child is in between sizes, we will size them up, which may require minor alterations at the parent/guardian's expense.

Our administration will place the order for your costume(s) and notify you to pick them up once they arrive.

I further understand if I complete this form after April 15th, 2026 (provided the costume is still available), I will be billed a late fee of $25.00 to cover all applicable expedited shipping charges to ensure the costume(s) arrives on time. I understand costume availablility and size availability is subject to change any time. If I order late, I will accept the next size up and alter accordingly.

- SHOWCASE REGISRATION FEE
  
  Toddler/Combo Class Students: $185 (includes costume, each additional class add $85 for costume fee)
  
  Level 1-4 Class Students: $225 (includes costume, each additional class participating will be charged for a costume fee only at a maximum of $125 per costume)
  
  What does this cover? A portion of this goes to applicable related expenses for producing a quality professional experience for our dancers and families. This fees includes a costume for one class, professional videography of the show with media download and commemorative t-shirt for each dancer.
  
  MULTIPLE CLASS COSTUME FEE
  
  If a student is participating in more than one class an additional Costume Fee will be billed to parents at a maximum of $95 (for toddler/combo classes) or a maximum of $125 for "Leveled Classes" per each additional participating class (Tax + Shipping Fees are included).
  
  Costume fees are subject to an additional late fee charge of $25.00 to cover expedited shipping if this Participation Form is completed after April 15th, 2026.
- CONSENT TO CHARGE CREDIT CARD | As the parent or legal guardian of the above-named participant, I hereby give my consent to charge my credit card currently on file in my Parent Portal (Jackrabbit) for the following: A the participation fee of $185.00 (Toddler/Kinder) or $225.00 (Leveled Classes) + any applicable costume fees (for additional classes) are non-refundable after 3/30/26.
  
  If student is later unable to participate, the parent/guardian acknowledges the refund policy for participation fees paid below:
  
  Cancellation requests received in writing to staff@avantidancecompany.com by 3/29/26 will receive a 100% refund less $25.00 administrative fee. However if the costume has been ordered and is not returnable, the costume fee will be deducted from the refund and dancer will still get their costume.
  
  Cancellation requests received 3/30/26 or later will not receive a refund.
