I have read and agree to abide by all tournament rules and general conduct expectations.All players must maintain good sportsmanship Substitutions are allowed during each game·All players are expected to demonstrate respectful behavior towards teammates, referees, opponents, and spectators at all times.Participants and attendees are expected to adhere to all Federal, State, and Local lawsParticipants and attendees must adhere to facility rulesFailure to adhere to rules and conduct may result in removal from the eventAvoid glass containers/bottlesAll teams must bring own equipment. Bats and balls will be checked prior to each game.Registration fee includes beer/alcohol permit for the team. Beer and alcohol ARE NOT PROVIDED / INCLUDED. Each team is responsible for their own beverages.
Waiver and Release of Liability
By submitting this form, the team and its members acknowledge that Fan of the Logo AZ LLC is not liable for any injuries or accidents occurring during the tournament. Each team is responsible for its own insurance coverage. All participants assume all risks associated with participation in this event.
• Participation is voluntary and entirely at your own risk.
• Fan of the Logo LLC, Fan of the Logo Arizona Chapter, their representatives, organizers, members, volunteers, sponsors, and City of Phoenix Parks & Recreation are not liable for any injury, accident, illness, loss, or damage to person or property occurring before, during, or after the event.
• All registered players, coaches, and attendees under your registration must comply with all event rules, state and local laws, and behave in a manner that ensures the safety and well-being of others. Failure to do so may result in removal from the event without a refund.
• You grant full permission to Fan of the Logo LLC and its affiliates to record, use, and distribute photographs, video footage, and audio recordings taken during the event for promotional, marketing, or commercial purposes, in both digital and print formats.
• You further authorize the use of your name, image, voice, and likeness in connection with event coverage, marketing, promotions, live streaming, or media produced for future events, without compensation, in perpetuity.
• You agree to hold harmless and indemnify Fan of the Logo LLC, Fan of the Logo Arizona Chapter, their representatives, members, volunteers, sponsors, and City of Phoenix Parks & Recreation from any claims arising from participation in the event or from the authorized use of your name, image, likeness, photograph, or video.
