Appointment & Discharge Policy
Dear Client,
At Community Behavioral Health Center ("CBHC"), we value the trust you have enlisted in us. To ensure the smooth operation of our services and to respect the time of all clients as well as our staff, we have established the following appointment policy:
1. Cancellation and Rescheduling:
We understand that circumstances may arise that require you to cancel or reschedule your appointment. However, to accommodate all clients effectively, we ask that you provide at least 48 hours' notice for any cancellations or rescheduling.
2. No-Shows and Late Cancellations:
If you miss an appointment or cancel/reschedule with less than 48 hours' notice, it is considered a late cancellation or no-show. Late arrival to appointments (15 minutes late or more) is also considered a no show/cancellation. The appointment will have to be rescheduled.
3. Repeated Violations (i.e. repeated cancellations or no-shows):
To maintain fairness and efficiency in our scheduling process, clients who cancel, reschedule, or miss three consecutive appointments or more than three appointments within a 12-month period without providing the required 48 hours' notice may be discharged from our services. We encourage you to maintain clear and open communication with your provider and/or office staff if there are circumstances that you would like us to consider. Consideration will be given at the discretion of the provider and in cases where there is evidence of extenuating circumstances.
4. Discharge Policy:
Clients who repeatedly violate this policy will be discharged from our practice. We will notify you in writing if this action becomes necessary.
We appreciate your understanding and cooperation. Our goal is to provide the best possible service to all clients, and adherence to this policy helps us achieve that.
