By enrolling in a Perfect Aesthetics & Spa membership, you agree to the following terms:
• Memberships are billed automatically each month to the card on file.
• A minimum 5-month commitment is required when enrolling.
• Monthly membership payments convert to treatment credit that may be applied toward services.
• Credits are non-transferable and may only be used by the enrolled member.
• Membership benefits and pricing apply only while the membership remains active and in good standing.
• Membership credits expire 12 months from the date issued if unused.
• Membership may be canceled after the initial 5-month commitment period.
• If a membership is canceled, any remaining membership credit must be used within the same calendar month of cancellation or it will expire.
• Early cancellation prior to the required 5-month commitment period will result in an early termination fee equal to two full months of the selected membership ($125, $199, or $299) and loss of all membership pricing benefits.
• If services were received using membership pricing prior to cancellation, the difference between the membership price and regular price may be charged to the card on file.
• Membership payments are non-refundable once billed.
• By enrolling, you authorize Perfect Aesthetics & Spa to automatically charge the card on file each month for the selected membership plan.
• Membership payments are authorized recurring charges and may not be disputed or charged back once billed according to membership terms.
• Any chargeback may result in immediate cancellation of membership benefits, forfeiture of membership credit, collection of any outstanding balances for services rendered, and enforcement of an early termination fee equal to two full months of the selected membership ($125, $199, or $299).
• All appointments must be booked online through our booking system.
• Appointments must be canceled or rescheduled at least 24 hours in advance.
• Appointments canceled with less than 24 hours notice will incur a 50% cancellation fee based on the scheduled treatment price.
• For membership clients, 50% of the treatment value will be deducted from the membership credit balance.
• For non-members, deposits will be forfeited and applied toward the cancellation fee.
• Clients who fail to attend an appointment without notice will be charged 50% of the scheduled treatment value.
• Clients arriving more than 15 minutes late may need to reschedule and the cancellation policy may apply.
