MSPACE SUMMER CAMP WAIVER & AGREEMENT

1. Participation & Assumption of Risk
I, the undersigned, as the parent or legal guardian of the minor child listed below, give permission for my child to participate in Mspace Summer Camp, including dance classes, rehearsals, choreography, physical conditioning, and related indoor or outdoor activities.
I understand that participation involves inherent risks, including but not limited to physical injury, falls, collisions, overexertion, or other accidents that may occur during dance and camp activities.
I voluntarily assume all risks associated with my child’s participation.

2. Medical Emergency Authorization
I authorize Mspace Entertainment Inc., its staff, and representatives to obtain emergency medical care for my child if I cannot be reached.
I understand that:
• Emergency services (including 911) may be called if necessary
• I am fully responsible for any medical expenses incurred
• I release Mspace from liability related to such medical care decisions

3. Health Disclosure
I confirm that I have disclosed any relevant medical conditions, injuries, allergies, or special needs that may affect my child’s participation.
I understand that failure to disclose such information may increase risk and releases Mspace from related liability.

4. Illness Policy
I agree that my child will not attend camp if they:
• Are experiencing symptoms of illness (fever, cough, etc.)
• Have a contagious condition
• Have been recently exposed to a serious infectious illness
I agree to inform Mspace if any such condition arises.

5. Media Release
I grant permission to Mspace Entertainment Inc. to photograph and record my child during classes and camp activities.
I understand that this content may be used for:
• Social media
• Marketing and promotional materials
• Website and advertisements
I waive any right to compensation or approval of such use.

6. Pickup Authorization
I understand that my child will only be released to:
• Myself (parent/guardian), or
• Individuals I have authorized in writing
Mspace is not responsible for children once they have been released to an authorized person.

7. Behavior Policy
I understand that respectful and safe behavior is required.
Mspace reserves the right to remove any child from camp if their behavior is:
• Disruptive
• Unsafe
• Harmful to others
No refunds will be issued in such cases.

8. Payment, Cancellation & Refund Policy
All payments for camp are non-refundable unless otherwise stated in writing.
Missed days, withdrawals, or schedule conflicts do not qualify for refunds or credits.

9. Release of Liability
I release and hold harmless Mspace Entertainment Inc., its owners, staff, contractors, and affiliates from any claims, liabilities, damages, or expenses arising from my child’s participation, including injury, illness, or property loss.

10. Indemnification
I agree to indemnify and hold harmless Mspace from any claims brought by or on behalf of my child arising from participation in camp activities.

11. Insurance Acknowledgment
I understand that Mspace does not provide medical insurance for participants and that I am responsible for maintaining my own coverage.
