New Food Allergy (Please note any allergies here and any interventions required to abate your child’s reaction. Severe allergies that require emergency medication administration, necessitate an individual healthcare plan (Projet d’Accueil Individualisé (PAI). The Health Centre team will be in touch with you to coordinate delivery of needed forms and documents.)
New Food Intolerance (Food intolerances must be supported by a medical certificate or food intolerance testing reports. The Health Centre team can then liaise with ISM’s executive chef, catering provider and nutritionist to support your child’s dietary needs.)
