Waiver and Release of Liability
In consideration of my admission onto property operated by Canine Evolution LLC or Twin Cities PSA Club, located at 11638 30th Ave NW, Princeton, MN 55371, 30209 116th Street NW, Princeton, MN 55371, or any off-site locations, I hereby release Twin Cities PSA Club, Canine Evolution LLC, and their owners, agents, consultants, and representatives (the “Released Parties”) from any and all actions, claims, or demands for personal injury, property damage, or loss, including injury to myself or my dog(s), which may arise out of my presence on the property and/or participation in any activity on said property or via phone consultation.
I understand that the safety of the dogs in the kennel, as well as any individual dog(s) with which I have contact—including my own—is my sole responsibility. I acknowledge that being around dogs, in a training environment, or participating/spectating in sporting events with my dog is inherently hazardous. I accept all risks associated with these activities and assume responsibility for the supervision of all children under my care at all times.
I understand that the Released Parties work with dogs that may have aggression or behavioral issues, and I agree to follow all procedures related to working around these and all other dogs. I agree that all dogs will remain leashed at all times unless instructed otherwise by the trainer. No children are to be left unsupervised at any time.
I also agree to pay for any costs associated with property damage or injuries caused by my negligence while participating in any activity. I acknowledge that I am under the policies and direction of the staff at Twin Cities PSA and that participation in any activity is entirely voluntary.
Photo and Video Release
I authorize the taking of videos and photographs of myself and/or my pet(s) by Twin Cities PSA Club and/or Canine Evolution LLC for professional and promotional purposes, including posting on Facebook, websites, and other online platforms. All media shall remain the property of Twin Cities PSA Club and/or Canine Evolution LLC and will be made available to me upon request at the discretion of management.
