** This form is only for local orders. International orders or jastip/proxies requests contact Kamis first.

Failure to adhere to this rule will result in a blacklist without notice.

We hope for your understanding and thank you for your support.

- Table number will be informed via E-mail
- Full payment through BCA, paid for items are non refundable.
- Orders that remain unpaid three days after the form closes will be canceled without notice.
- You will receive invoice of your order via email (maximum 7x24 hours after the order is received) please be patient.
- Orders that are failed to pick will be back to inventory within 30 working days, and without refund
