EDUCATION RECORDS: I authorize the release of my student’s education records to Bay Scholars for the purpose of evaluating and administering the Bay Scholars Scholarship Program. This may include: High School Placement Test (HSPT) scores, transcripts, college acceptances and matriculation, financial aid information, and demographic data.

I have read and agree to the FULL Bay Scholars Release and Consent Documents.

I do not give permission for Bay Scholars to use my student’s name, likeness, voice, and biographical information for educational and promotional purposes in any format.
