Registration Agreement
The implantation of 24PetWatch microchips is offered free of charge to pet owners, subject to availability, on a first-come, first-served basis. Donations are appreciated. Pre-registration does not guarantee microchip implantation, as supplies are limited. The pet owner must be present at the time of service.
Pet Handling Requirements
- Dogs must be properly restrained with a well-fitted leash and harness/collar or secured in a humane carrier.
- Cats must be secured in a humane carrier.
- Puppies, kittens, and senior pets should be handled with care to prevent stress and potential spread of illness.
- Pets that are reactive or prone to high stress/anxiety in veterinary settings should be muzzled or kept securely in a humane carrier to prevent injury to staff, volunteers, or other attendees. You may be instructed to keep your pet confined to your vehicle until it is your turn for service.
- To ensure pet and attendee safety, you may be instructed to hold your pet in your arms so they are not on the ground/floor, place them in a carrier, or keep them in your vehicle until staff and volunteers are ready for your pet.
Health Restrictions
Animals exhibiting diarrhea, lethargy, coughing, vomiting, ocular or nasal discharge, or any other signs of illness are not permitted to attend.
Service Refusal & Liability
Event volunteers and staff reserve the right to refuse service if it is deemed in the best interest of the animal, staff, volunteers, or other attendees. Owners are liable for any injury or illness their pet may cause to event staff, volunteers, other attendees, or animals.
Microchip Registration & Contact Consent Pet owner and pet information must be provided to register the microchip with the Humane Society of St. Joseph County and 24PetWatch. Your contact information, including email, may be used to:
- Notify you about free or budget-friendly pet services, adoption promotions, donation appeals, and events.
- Share exclusive offers, promotions, and updates from 24PetWatch regarding microchip and pet insurance services.
- By signing this agreement, you consent to Pethealth Services (USA) Inc., Pethealth Services Inc., PTZ Insurance Services Ltd., and PTZ Insurance Agency Ltd. contacting you via: Commercial electronic messages
Automated phone calls, pre-recorded messages, or text messages. These communications are not required for the purchase of any goods or services.
- You understand that if you choose not to provide your consent, you will not receive electronic enrollment notification regarding free lost pet services which includes being contacted with information in the event that your pet goes missing.
- You may withdraw your consent at any time.
Liability Release
I understand that participation in the Humane Society of St. Joseph County’s (“HSSJC”) microchip clinic involves certain inherent risks to my animal, including but not limited to minor pain, bleeding, swelling, infection, migration of the microchip, equipment malfunction, or other unforeseen complications.
I certify that I am the legal owner or authorized agent of the animal presented for microchipping and that my animal is healthy enough to safely receive a microchip at this time. I understand that HSSJC staff and volunteers will perform the procedure to the best of their ability but are not providing full veterinary services or a comprehensive health evaluation.
I acknowledge that the microchip implantation process involves inserting a chip beneath my animal’s skin using a needle, and while generally safe, complications may occur. I understand that it is my responsibility to monitor my animal following the procedure and to seek veterinary care at my own expense if any complications arise.
I understand that the Humane Society of St. Joseph County will register my animal’s microchip with 24PetWatch at the time of implantation. I further acknowledge that it is my responsibility to ensure that all contact and ownership information remains accurate and up to date. I agree to promptly update my information directly with 24PetWatch and the Humane Society of St. Joseph County in the event of any changes. HSSJC is not responsible for maintaining, updating, or verifying the accuracy of registration information after initial registration.
By participating in this clinic, I voluntarily assume all risks associated with the microchip procedure.
To the fullest extent permitted by law, I hereby release, waive, and discharge the Humane Society of St. Joseph County, its officers, directors, employees, volunteers, partners, and agents from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur to my animal, myself, or my property as a result of participation in this clinic.
I agree to indemnify and hold harmless HSSJC from any claims resulting from my participation or my animal’s behavior during the clinic.
Consent Requirement
If you do not agree to these terms, your pet will not be microchipped.
