    I hereby acknowledge that the above named player has no pre-existing injuries/illnesses, heartconditions, or asthma and is in good physical health to participate in the Arizona Western College Clinic. If these conditions exist, the above named player has a doctor’s clearance to participate in the Arizona Western College Clinic. I hereby authorize the staff of the Arizona Western College Matador Clinic to act for me according to their best judgment in any emergency situation requiring medical attention. I hereby acknowledge that participation in this clinic, as in any sport, may result in accidents and/or injuries. Even though I know there are risks involved, I still give my approval for the above named player to participate in all clinic activities. I expressly assume all risks and hazards incidental to such participation, and do hereby waive, release, absolve, indemnify and agree to hold harmless the Arizona Western College Matador Clinic, its staff, suppliers, sponsors, participants and Arizona Western College for any claim arising out of injury or illness to said player regardless the cause. SB1521 Youth Concussion Bill: Any participant under the age of 18 will be removed from the event byan athletic trainer, coach, or official if they present with concussion symptoms. The participant may return to participation the following day if they have written clearance from a licensed physician, physician assistant, or athletic trainer.
