Welcome to stage 2 of the enrollment process for Great Revelations Academy.

Important Instructions:
- Please note that when you are scheduling a stage 2 appointment, you must have already passed stage 1 and been instructed through email by the school office to proceed. (Kg through 12th grade)
- Fill in all required fields
- Review the form before submitting.
- Click "submit" to confirm your appointment.
Accepted Students have the options for in-person registration in either Dearborn or Dearborn Heights campus.

PRE-SCHOOL REQUIREMENTS:

Dearborn Heights Campus Pre-K Registration will be available on Tuesdays and Thursdays from 10 am to 1:30 pm only

Enrollment Eligibility:

Pre-K 3 Enrollment Criteria:

Children must turn 3 years old on or before October 1st to be eligible for enrollment in the Pre-K 3 classroom.

Pre-K 4 Enrollment Criteria:

Children must turn 4 years old on or before December 1st to be eligible for enrollment in the Pre-K 4 classroom.

FORMS & DOCUMENTATIONS:

As you come in during your scheduled time slot, please bring the following documentations:

Driver's license
Proof of residence (DTE Bill)
Birth certificate
Health Appraisal & Immunization Record
First payment (cash, check, Zelle)
