Terms and Conditions* This field is required.I, the parent/guardian of the participant listed below, give permission for my child to participate in the World Class Training Camp hosted at Rumble City Boxing Gym. I understand that participation in sports including basketball, boxing, strength training, and recreational activities involve inherent risks, including but not limited to injury, illness, or accidents. I acknowledge and agree to the following: 1. I release and hold harmless World Class Training Camp, its owners, coaches, staff, volunteers, and facility partners from any liability, claims, or damages arising from participation. 2. I certify that my child is physically fit and able to participate in athletic activities. 3. I agree to inform camp staff of any medical conditions, allergies, or special needs prior to participation. 4. I understand that all boxing participants must be registered with USA Boxing for insurance coverage. 5. I give permission for emergency medical treatment if I cannot be reached. 6. I understand that camp fees are non-refundable. 7. I give permission for photos/videos of my child to be used for promotional purposes.
