Nature of Services Provided
Trusted Transitions is a senior living placement and referral support service that assists individuals and families in identifying senior housing and care options, including: Independent living communities, Assisted living communities, Memory care communities, Residential care homes, Skilled nursing facilities, and/or other senior housing resources. Trusted Transitions provides information, guidance, and referral support only. Trusted Transitions does not: own, operate, manage, supervise, control, employ staff at, provide services within, any senior living community or care facility. Trusted Transitions does not provide: medical advice, healthcare services, nursing services, legal advice, financial advice, or insurance advice. Any decisions regarding care, placement, treatment, or services must be made independently by the client and/or their family in consultation with appropriate licensed professionals.
California Residential Care Facility Referral Disclosure: Trusted Transitions Senior Care Advisors, LLC provides referral support for Residential Care Facilities (RCFEs), assisted living communities, memory care communities, independent living communities and other senior housing options within California. Trusted Transitions is a referral and advisory resource only and is not a licensed care provider, operator, or healthcare entity. Trusted Transitions does not evaluate clinical care delivery within facilities and does not guarantee regulatory standing, staffing adequacy, admission eligibility, or continued residency. Clients are encouraged to verify licensing status and regulatory history directly through the California Department of Social Services Community Care Licensing Division (CCLD) and other applicable regulatory agencies
Information Accuracy and Limitations
Recommendations provided by Trusted Transitions are based on information available at the time of referral, including: information supplied by care communities, publicly available sources, third-party sources, regulatory agencies, and/or client-provided information. This information may change at any time without notice. Such changes may include, but are not limited to: ownership changes, management changes, staffing changes, licensing status, regulatory compliance status, complaint history, inspection results, policies and procedures, pricing structures, availability of services, and/or level-of-care capabilities. Trusted Transitions makes no guarantees, representations, or warranties regarding the quality, safety, services, availability, regulatory standing, or performance of any facility or provider.
Licensing and Regulatory Status Verification Responsibility
Licensing status, inspection reports, complaint history, citations, enforcement actions, and regulatory compliance information for Residential Care Facilities for the Elderly (RCFEs) and other senior living providers may change at any time. Clients acknowledge responsibility for independently reviewing facility licensing information through appropriate regulatory agencies including: California Department of Social Services & Community Care Licensing Division (CCLD). Trusted Transitions does not guarantee the completeness, accuracy, or current status of regulatory information obtained from third-party sources.
Independent Decision-Making Acknowledgment
All placement decisions are made solely and independently by the client and/or their representatives. By using our services, you acknowledge that: You are responsible for: touring communities, verifying services, confirming availability, confirming, pricing, reviewing contracts, evaluating appropriateness of care and/or consulting physicians or licensed professionals when necessary. Trusted Transitions provides informational support only and does not make placement decisions on behalf of clients.
Client Responsibility for Facility Evaluation
Clients acknowledge responsibility for personally evaluating each senior living community prior to making placement decisions, including reviewing: facility agreements, resident rights documents, service plans, pricing structures, disclosure statements, licensing history and care capabilities Trusted Transitions does not interpret or validate facility contracts.
No Monitoring of Care After Placement
Trusted Transitions does not supervise, monitor, evaluate, or oversee care services provided by any community after placement. Trusted Transitions is not responsible for: care outcomes, supervision levels, staffing adequacy, changes in condition, changes in services, discharge decisions, facility transfers, regulatory issues, complaints or investigations, and/or incidents occurring after placement. Ongoing evaluation of care remains the responsibility of the client and/or their representatives.
No Guarantee of Admission or Continued Residency
Trusted Transitions does not guarantee admission to any senior living community. Admission decisions are made solely by each individual community based on their own policies, assessments, licensing limitations, and regulatory requirements. Trusted Transitions does not guarantee continued residency after admission and is not responsible for transfer or discharge decisions made by any facility.
Limitation of Liability
By using this website or any services provided by Trusted Transitions, you agree that Trusted Transitions Senior Care Advisors, LLC, including its: owners, employees, contractors, representatives, affiliates, and referral partners shall not be liable for any outcomes related to placement decisions or referrals. Trusted Transitions is not responsible for determining whether a facility can safely meet the present or future care needs of any client. This includes, but is not limited to: falls, injuries, accidents, illness, medical treatment decisions, lack of, medical treatment, negligence by facility staff, misconduct by facility staff, regulatory violations, licensing issues, complaints or citations, changes in staffing, ownership transitions, service reductions, changes in policies or procedures, financial disputes, discharge decisions, relocation decisions, facility closures, and/or changes in quality of care. You expressly release Trusted Transitions from any liability arising from or related to placement recommendations or referral assistance.
Third-Party Facility Responsibility Disclaimer
Senior living communities operate independently from Trusted Transitions. Trusted Transitions does not: verify clinical appropriateness, guarantee admission eligibility, guarantee, continued residency, guarantee service availability, guarantee staffing levels, and/or guarantee regulatory compliance. Responsibility for evaluating facilities remains with the client.
Compensation Disclosure
Trusted Transitions may receive compensation from participating senior living communities or providers if a placement occurs. This compensation: does not increase the cost to the client, does not influence recommendations, or supports continuation of no-cost placement services to families. Clients are never obligated to select a referred provider.
California Required Referral Relationship Transparency
Trusted Transitions maintains professional relationships with certain senior living communities and service providers. Some providers may compensate Trusted Transitions if a client chooses to move into their community following a referral. Clients are under no obligation to select any referred provider and may independently choose any community regardless of referral relationship status. Referral compensation does not affect the cost of residency for clients. Trusted Transitions makes recommendations based on client needs, preferences, availability, and information provided at the time of referral.
No Professional Relationship Created
Use of this website or services does not create: a medical provider relationship, a legal advisory relationship, a fiduciary relationship, and/or a case management agreement. Trusted Transitions provides referral guidance only.
No Fiduciary or Clinical Assessment Relationship
Trusted Transitions does not perform medical assessments, clinical eligibility determinations, or financial qualification evaluations for admission into any senior living community. Trusted Transitions does not determine appropriateness of placement from a medical, psychiatric, or financial eligibility standpoint. Clients acknowledge responsibility for obtaining independent evaluations from licensed professionals when appropriate.

All placement decisions remain the sole responsibility of the client and/or their representatives.
Updates to Terms
Trusted Transitions reserves the right to modify these Terms of Use, Disclosures & Service Limitations at any time without prior notice. Continued use of services constitutes acceptance of any updates.
