Please read and agree to the following terms. These are required to receive a tattoo. Your acknowledgement signifies your agreement and understanding of these terms. If you arrive to your appointment and it is indicated that you are in violation of the requirements, your appointment will be cancelled and a 50% fee will be charged.

If I have diabetes, cancer, heart condition or am currently undergoing treatment for cancer, I will provide my doctor's approval. I understand that my results may be affected by these conditions.
I have not used Accutane in the past year.
I will avoid sun and tanning two weeks prior to my appointment.
I will follow the aftercare instructions provided to me and understand that failing to do so WILL impact my healed results.
Clients arriving more than 15 minutes late may forfeit their appointment and will be considered a no-show. We require AT LEAST 48 hours notice when cancelling or rescheduling ANY appointment to avoid being charged 50% of the full price of the service.
Provide a SPECIFIC description of the tattoo(s) you are requesting & include any specific names, dates and flowers if necessary (if you sent initial inspo on instagram, please resubmit everything here).
