Payment is required to secure all appointments. An invoice will be sent once your appointment request has been reviewed and confirmed. Appointment time slots are not guaranteed until payment has been received.

Cancellation Policy:

If you need to cancel or reschedule, please notify us at least 24 hours in advance. Missed appointments or same-day cancellations may result in a cancellation fee.

Service Notice:

Eleve Testing Solutions LLC is a mobile DNA collection service. We collect DNA samples and send them to a certified laboratory for analysis. We do not process DNA results. All results are released by the laboratory.
