To be eligible all for the program, all of the following must apply: (Check each)

Be at least 16 years of age before starting the program (June 1)
Completed at least your sophomore year of high school by June 1 and have at least a C average with excellent school attendance
Be recommended to the program by a teacher, counselor or PHCC member in writing
Obtain parental permission with a release for the PHCC (form at end of application)
Adhere strictly to the attendance and employment regulations required by the employer
Adhere strictly to the TN Child Labor Laws ( A copy will be provided to you)
