    2026 PASEO VERDE SUMMER CAMP  REGISTRATION FORM

    This registration form is for childcare, NOT for summer school.

      Tuesday May 26 - Friday July 24, 2026

    Before starting this form, please have a copy of your child's
    immunization records available to upload or to take a picture.  Previous submissions are NOT on file and a new copy is REQUIRED to complete this registration.

    The City of Peoria is currently taking registrations for 2026 Summer Camp (childcare).  This program is licensed through the Department of Health Services, and is available from 6:00AM to 6:00PM Monday - Friday.  Please be aware that care for June 19 and July 3rd will be available ONLY at these schools:

     Cheyenne Elementary:  11803 N 87th Ave
     Sunset Heights Elementary: 9687 W Adam Ave

    Weekly attendance fees for the program will be automatically charged every Monday at the start of each week based on the scheduled attendance you select.  A $30.00 non-refundable registration fee per child will be charged for NEW childcare particpants and $10.00 per child for CURRENT 2025-26 AMPM particpants or returning 2025 Summer Camp participants. This fee will be processed when your registration is processed by our staff.  We do NOT offer multi-child discounts. 

    Your finance information is REQUIRED for automatic payment and must be added to your Peoria Recreation account within 12 hours after submitting this form. Directions on how to add your finance information are listed at the end of this registration.   Please take a picture of these directions or write them down before submitting this registration form.

    Administration business hours are Monday through Thursday, 7AM to 6PM except holidays. CLOSED on Fridays.  Please call 623-773-7139 or 623-773-7505 with any questions regarding registration or pricing.

    Program capacity at each school is dependent on available space and staffing.

    Please check the days needed for childcare each week.   You must choose all days/weeks your child will attend.  
     No credit back for sick/missed days or early pick-ups. 

    Drop In Schedule is NOT an option.  

    RATES:  Weekly attendance fees are a Flat Rate, NOT a daily rate.

    Part Time Option: 1-3 days: $115.00, whether you select 1,2 or 3 days a week. 

    Full Time Option: 4-5 days:  $185.00, whether you select 4 or 5 days a week. 

     

    UNSCHEDULED Days of attendance:  $50.00 per day, per child.  If you select Mon, Tues & Wed on the registration form and attend Mon, Tuesday,Wednesday & Friday, you will be charged for your (3) regular scheduled days of part-time care PLUS $50 for attending Friday, an unscheduled day of attendance.

If yes, it is the parent/guardian's responsibility to ensure the billing department has received your DES authorization PRIOR to registering for the Summer Camp Program.  Your account will be charged full fees until DES approval has been received by the billing department.  Child must be must be approved at the Summer Camp site chosen.  Please call 623-773-7505 or 623-773-7139 to verify DES approval has been received.*

  • About Me Questionnaire

    This confidential information will be used to help your child care provider support the growth and development of your child while creating a safe, stable and healthy environment for all children. By providing complete information about your child, you will be assisting us in creating a positive experience for your child while in our care.  Confidentiality is a vital component in the child care setting.  Therefore, we will only share this questionnaire with the child care director, owner and your child's primary teacher unless approved by a parent/guardian.

    Instructions: A parent/guardian must complete the Best of Care form, and it must be on file at the program on/or before your child’s first day of attendance.  Additionally, the questionnaire should be updated when significant changes occur in the child's care or annually. 

    In order to better service citizens who require modification, Please complete the Inclusion Service Form. This needs to be 1-2 weeks prior to the first day of attendance.

    On behalf of myself, my household members, and as parent and lawful guardian of my minor child, I hereby give permission for my child to receive childcare services at the City of Peoria Summer Camp program. My child and I are familiar with, and knowingly and voluntarily accept, any and all risks associated with childcare on a school campus. I acknowledge that my child’s participation in this program is voluntary and is not part of any regular school curriculum.

    I specifically assume all risks and hazards associated with my child’s participation in the City of Peoria Summer Camp program. I understand that my child will be associating with staff and other children and may be exposed to viruses and diseases, through my child’s participation in the City of Peoria Summer Camp program. I understand and voluntarily assume the risk that my child may acquire a virus or disease and subsequently be transmitted from my child to me, my family, and members of my household.

    I certify that my child is in good health, has no fever, and has no current issues that make it unsafe for my child to participate in the City of Peoria Summer Camp program, which will not have a medical professional on-site. I will notify the school and not send my child to the City of Peoria Summer Camp program if my child develops a fever or illness.

    To the fullest extent permitted by law, I hereby agree to waive, release, and discharge any and all claims, causes of action, damages, and rights of any kind, including serious injury or death, against: the City of Peoria, the school where my child attends, the Peoria Unified School District, its insurers, the district’s governing board, and all of their respective employees, agents, representatives, and volunteers (the “Released Parties”) arising from or relating in any way to my child’s participation in the City of Peoria Summer Camp program.

  • To Add your Finance Information:  Be sure you are using a desktop or laptop computer and Google Chrome as your internet browser.  After you click on SUBMIT below, you MUST log into your City of Peoria Recreation account and input your required finance information.  To do this, you will log into your recreation account, click the primary account holder (parent or guardian only, NOT the child), then scroll down and look for the FINANCE INFO line.  Click on the NEW button. After the screen opens, be sure you check the box next to the Default button at the top left.  Complete the information and click Save.
     

    Failure to submit your financial information within 12 hours  WILL result in the removal from the Summer Camp Program.
