El motivo de este formulario es para dejarle saber sobre el evento "Noche de Patinaje" que tendremos el Viernes 27 de Marzo en Ice Land en la 6 Tennis Ct. Hamilton, NJ 08619 con los jovenes. Al firmar este formulario, usted padre/madre/guardian esta de acuerdo de dejar participar a su(s) hijos(as) al evento con los jovenes de Ministerios Fraternidad Cristiana de Trenton, NJ. Si eres mayor de edad, necesitas firmar este formulario tambien.**Este evento lleva consigo varios retos y riesgo de golpe.** Nota: La iglesia no se hace responsable por cualquier golpe o objetos olvidados. Cualquier niño menos de 13 años necesita ser acompañado con su padre o guardian. ¡Muchas gracias por entender, bendiciones!

Costo por persona si va a patinar: $12.00. Habra oportunidad de comprar comida en el lugar.

The reason for this form is to let you know about the "Ice Skating Night" event that we are having on Friday March 27th at Ice Land at 6 Tennis Ct. Hamilton, NJ 08619 with our youth. By signing this form, you the parent/guardian agrees to let your child(ren) participate in the event with the youth of Ministerios Fraternidad Cristiana Trenton, NJ. Even if you are of legal age, you need to sign this form.**This event carries with it several challenges and risk of injuries.** Note: The church is not responsible for any injuries or forgotten items.
Any child under the age of 13 needs to be accompanied by a parent or guardian. Thank you very much for understanding, blessings!

Cost per person if you decide to skate: $12.00. There will be an opportunity to buy food at the facility.
