This form is used by box teams to record or request approval of an affiliation for the 2026 season in compliance with Ontario Lacrosse Association Regulations.

It must be completed by a Designated Representative of the Club/Team. By submitting this form, you confirm that you are authorized to act on behalf of the teams involved, that the information provided is accurate, and that appropriate contact information has been provided to receive official affiliation decisions.
